Whole day £440/day (£55/hour)

Half day (4.0 hours) £240. Latest finish 1pm.

Day 8.30am - 5pm or pro rata equivalent, (eg 9am - 5.30pm) to include 1/2 hour for lunch.
Additional day rate outside 8.30am-5pm @ £65 per hour.

Visits £20 each

Off site on call rate £27.50hour. Hourly call in £65/hour. Min call in 1 hour followed by half hourly additions.
Work after 10pm £95/hour.

Mileage at 60p per mile to include travel to and from practice, daily to and from place of work and in-work travel.
Mileage rate includes an element of cost for travel time previously charged separately.
Distant locum work either costed on car travel or flight costs plus car hire during assignment.

Payment 1 week in arrears. Basic rate to be in hand at the end of each working week (5.30pm Friday) to ensure subsequent employment.
Signed letter of agreement required to confirm booking.
Cancellation or alteration fee after agreement £150 per day (or part of).
Non-fulfillment of agreed terms is grounds for termination with 24 hours' notice.
This document is a form of agreed terms and constitutes substantive basis for contract of employment.
